Allemand Außerirdische überfallen die Erde und entführen Frauen, die nackt als Geiseln gehalten werden. Ziel des Spiels ist es, die Invasoren zu stoppen und somit die Welt zu retten. Dabei trifft man unter anderem auf zu Warzenschweinen mutierte Polizisten des LAPD ("Pig Cops") sowie auf reptilienähnliche Gegner. Neben aus anderen Spielen bekannten Waffen wie zum Beispiel der Pump Gun oder dem Raketenwerfer, gibt es in Duke Nukem 3D auch innovative Waffen wie Rohrbomben, den "Shrinker" (zum Schrumpfen und anschließenden Zertreten der Gegner) oder den "Freezer" (zum Einfrieren der Gegner). Darüber hinaus konnte sich der Spieler mit Hilfe eines Jetpacks fliegend fortbewegen.

Français De retour sur Terre après ses aventures spatiales dans Duke Nukem II, Duke Nukem survole Los Angeles lorsque son vaisseau est abattu par des extraterrestres. Une centrale militaire l'informe qu'ils ont besoin d'aide et que des êtres qui ne viennent pas de la Terre sèment la panique dans la ville. Ces derniers ont envahi le monde pour asservir l'humanité, piller ses ressources minières et s'emparer des humaines. O jogador possui algumas habilidades especiais, tais não existiam nos jogos da época. Era possível se abaixar ou pular, nadar e voar, além de poder mirar verticalmente nos inimigos/objetos no cenário utilizando mouse ou teclado. Também é utilizado um sistema de inventário semelhante ao de Hexen e Heretic, onde alguns itens poderiam ser armazenados para serem usados apenas quando fora necessário.
