Anglais No rest for the wicked - that's something Reimu Hakurei, a young girl guarding the medieval Japanese Hakurei shrine, has understood during her eventful life. Demons from a parallel dimension have chosen her shrine as an entry to our earth, from which they begin their invasions. This time they appeared in the mountain cave, and Reimu must stop them before they conquer the entire world. But this time, she is not alone...

The fifth entry in the Tōhō series follows the traditional vertically scrolling shooter template first introduced in the second game. Enemies, level design, collecting weapon upgrades, using a limited amount of powerful "bombs" - all the elements from the other Tōhō games are present here. Like in the previous entry, the witch Marisa, Reimu's rival, is a playable character; in Tōhō: Kaikidan, two more characters "join the club", the demonic (though looking like young cute girls) Mima and Yuka, who starred as boss enemies in the fourth game.

Espagnol La mecánica de "Mystic Square" es bastante similar a la de sus predecesores, es otro juego de disparos (desde una nave) en el cual se emplea un sistema de bonificación parecido al de "Lotus Land Story", en el que se recompensa a los jugadores cuanto más arriesgados sean sus movimientos (por ejemplo, se reciben puntos si se va a recoger items en la parte superior de la pantalla), dichos puntos obtenidos, caen, y el jugador tiene que recogerlos al tiempo que si disparando.
En este juego, los jugadores tienen la oportunidad de elegir entre cuatro protagonistas para completar el juego, dos de ellas antiguas enemigas de los dos juegos anteriores. Mima y Yuuka Kazami se unen a las dos representantes de la serie, Reimu Hakurei y Marisa Kirisame para embarcarse en un viaje hacia las profundidades de Makai.
Aunque se amplió el listado de personajes jugables, se eliminó la posibilidad de elegir sus tipos de ataque específicos.

Français Alors que nos filles sont encore en train de se remettre de l'arrivée des youkai, des démons du royaume de Makai commencent maintenant à essayer d'envahir. Une fois de plus, Reimu décide de couper l'invasion à la source. Chacune pour ses propres raisons, Marisa, Mima et Yuuka décident également qu'il est dans leur intérêt d'enquêter.
