Allemand Freddy Pharkas war in den 1880er Jahren einer der Schnellsten im amerikanischen Westen. Bei einer bestimmten Schießerei mit einem Gesetzlosen namens Kenny war er jedoch waffentechnisch unterlegen und Freddy wurde sein Ohr abgeschossen. Dieses traumatische Erlebnis veranlasste Freddy, seine Pistolen aufzugeben und sich an der Schule für Pharmakologie einzuschreiben. Die Erinnerungen an seine Vergangenheit verdrängend, kommt Freddy in die Stadt Coarse Gold mit dem Traum, eine eigene Apotheke zu besitzen und zu betreiben. In Zusammenarbeit mit dem Arzt der Stadt ist Freddy in der Lage, Rezepte auszufüllen, Mixturen zu mischen und einfache Symptome zu diagnostizieren. Die Geschäfte unter den Stadtbewohnern sind jedoch rege, die guten Zeiten sind nicht von Dauer, da die Stadtbewohner plötzlich aus der Stadt vertrieben werden und die Geschäfte abgeschottet und mit Brettern vernagelt werden. Freddy muss sich mit den Problemen der Stadt auseinandersetzen und das große Ganze aufdecken oder riskieren, einen schicken Laden in einer leeren Stadt zu haben und alle seine Kunden zu verlieren.

Mais un jour il tomba sur un hors-la-loi du nom de Kenny, celui-ci lui arracha une oreille d'une balle de revolver durant un duel. Freddy décida alors de laisser tomber le tir et les duels, et décida de devenir pharmacien. Au bout de cinq ans il obtient son diplôme et s'installe à Coarsegold où il ouvre une pharmacie. Au début de l'histoire, narrée par un vieil homme nommé Whittlin Willie, Freddy découvre que quelqu'un veut Coarsegold en ville fantôme. En plus de pratiquer la pharmacologie, il devra préserver la ville de toute sorte de dangers, tels que l'eau empoisonnée, les incendies criminels, les parieurs, et même les flatulences !
